Problem in case creation in 'Case Worker'--->'Agent Dashboard'

Hi,

We have installed Oracle EBS 12.1.1 vision instance, and facing following problems in 'Agent Dashboard' screen of 'Case Manager' responsibility---

(1) While selecting a case type in 'Create Case' LOV and pressing 'Go' button, 'error page' is coming and error stack is coming.

(2) After searching party and creating case for that,  'error page' is coming and error stack is coming.

We are stuck on these points. Find attached screenshots for this.
