Tasks with Multiple Assignments

Hi,
We are encountering an issue where the user is reporting that multiple task assignments are present without them doing anything deliberate to cause this. Once there are multiple resources assigned to the task then the task is unable to transition to the next task status.

What ways can multiple assignments for a task occur? I have been able to duplicate from Resource Tab or SR form, but are there other ways to have multiple assignments?
Is there a way to disable this feature?
Is the task transition not being able to occur standard oracle functionality?
