Optach issue "Could not load program /lib/crt0_r.o:"

We are applying below patches on 10.1.2.3 forms and report oracle home.

5705981
7230832
12379038

and getting the same error for all patches.

Log output is as below:

Running make for target ias_install OBJECT_MODE=32.

OPatch found the word "error" in stderr, please look into it.
You may have to run the make again.

We have Rebuild Forms and Reports executables:

cd $ORACLE_HOME/forms/lib32
Note: if this directory does not exist: cd $ORACLE_HOME/forms/lib
$ make -f ins_forms.mk install
