What is the best approach to be followed for performing upgrade from Oracle Apps 11.5.10.2 to Oracle

Hi,
I wanted to get the best approach to be followed for performing upgrade from Oracle Apps 11.5.10.2 to Oracle R12.1.3.
Need the appproach to be followed during upgrade for the following
1) Conversions components
2) Interface Components
3) Reports
 How to convert a normal report to multi-org report
 What are the key things to remember while upgrading XML Publisher Reports
4) Forms
5) Workflows
6) OA Framework pages.
