Printing Long and Short Attachments

I'm struggling to get all the attachments to print out using the Oracle seeded Standard PO Data Definition and BI Publisher via an RTF template.  Here are the two issues I've run into:

The attachement text is included in the xml output, but

1)  Header Attachments -  I can only get one of each the Long or Short Attachment text to print.  I've tried using the for each:HEADER_ATTACHMENTS but then nothing prints at all.

2) Line Attachments - none of these are printing at the line level at all.  I've also tried using the for each:LINE_ATTACHMENTS without anything changing.
