Screen renders poorly

What would make an interactive version render poorly in 9.1?

We have multiple versions of PO Entry, P4310.

One version of this attached to one menu has additional (non-sensical!) spaces and such in the PO header portion of the screen.

I have created a new version.

I have aded a new task and deleted the old one.

I have cleared menu cache.

I have closed my browser and come back in. No change.

Any ideas?
