11.5.10 to R12.1.1 (driver 6678700) - post upgrade concurrent request failures

After running the R12.1.1 upgrade driver u6678700.drv and then running rapidwiz to configure the system, I have checked the concurrent request failures from tasks run by the upgrade.
Quite a number have failed with an error like this ..
"FDPSTP failed due to ORA-20100: Error: FND_FILE failure. Unable to create file, oKLREPUP.tmp in the directory, /usr/tmp."

In our case UTL_FILE_DIR is set to /usr/tmp/ORACLE_SID and APPLPTMP in the 11i system is set to the same.

I have re-run the upgrade and before running the upgrade driver I have manually set APPLPTMP to /usr/tmp/ORACLE_SID, but I still get the same failures.
