A requisition submitted for approval does not go the approver for approval. 3103: Attribute 'DOCUME

Hi,
When a requisition is submitted for approval, the status goes to 'In Process'  and the notificaiotn is not sent the approver.
And in the 'Action history', I see only one action, i.e submitted.
In the 'view Approval through workflow', 'reserve document' is in error.
And also, we see the below error.

3103: Attribute 'DOCUMENT_ID' does not exist for item 'REQAPPRV/173140-261881'.
