Orphan records-Note Id 169677.1- How to Cleanup a move order

Hi

I am working on an issue where in a particuler Sales Order gets stucked in "Pending Transaction form".

Then i apply the script as per the metalink Note ID 169677.1.

when i try to perform the same in Test Instance the transaction are successfully executed and not stucked in "Pending Transaction form"

May i know the following:

1- What is  the causes of the above issue?

2- In General, What are the causes of Orphan records?
