Patch set not applied properly.

Oracle = Oracle Database 11g Enterprise Edition Release 11.2.0.3.0 - 64bit Production
OS = Linux 6
RAC = 2 nodes
OPatch Version = 11.2.0.3.4
Download Patch: p16742216_112030_Linux-x86-64.zip( include patch 16619898 and 16619892)
ASM

command used to apply the patch set:
sudo $GRID_HOME/OPatch/opatch auto /opt/app/oracle_software/11.2/patch -ocmrf /opt/app/11.2.0/grid/OPatch/ocm/bin/ocm.rsp

I applied the "Oracle Grid Infrastructure Patch Set Update 11.2.0.3.7 (Includes Database PSU 11.2.0.3.7) - 16742216.
As I am watching the screen, the output indicated that the ORACLE_HOME and GRID_HOME patched successfully but when I ran the lsinventory, I found that it did not apply the entire patch set. I will attach the screen out for each nodes. I can understand that one option is to rollback the patch and reapply it, but I am confused as to why it did not apply the entire Patch Set.
