Why can't I create a sales order line in R12?

We are starting are upgrade project from 11.5.10.2 to 12.3.1. We have a new R12 instance that was recently upgraded to R12. I am not able to create a sales order line. Please refer to the attached navigation path and screenshots. The Sales Order Header is created; however, the sales order line is not saved/booked. What am I missing?

(N) US OE IT ACCESS > Orders, Returns > Sales Orders > Enter Customer, Customer PO, Order Type, Ship To Location

(T) Line > Enter Item, Quantity > (B) Book > Sales Order Number 511423005 was created but Order Line Status is still blank (Expect it to be Booked) > (I) Save Order Line Status is still blank (expect it to be Entered)
