Why can't I create items in R12?

We have a new R12 instance that was upgraded from 11.5.10.2 to 12.1.3. I have been performing basuc funcional testing on this instance in preparation for our CRP next week. I am not able to create an item. Please refer to the attached screenshots

(N) US INVENTORY ITEM ACCESS > Items > Master Items > When prompted, select Organization = WD > Enter Item

Number, Description > (M) Tools > Copy From > Select Template = Assembly > (B) Apply > (B) Done > Enter Each on

the Unit of Measure >  (I) Save > usually I will get a message that the item was saved; this time there is no message, and

 the item is not saved/created
