Workflow prompting for value

I am attempting to get Workflow to prompt me for a value/statement when the STATUS changes on our SRs. Currently we have a STATUS='Waiting on Customer'. I am trying to have workflow ask the user the reason for the status change an then write that

particular reason out as an Activity on the SR. Has anyone successfullly accomplished something like this?
