Cost Component ID = 0 and Analysis Code is NULL

After running all the actual cost process, cost update, preprocessor, and create accounting in draft mode, I am getting some records where the cost component ID is 0 and the analysis code is null.  Most of the records are purchasing transactions but some inventory, order management and production transactions have this problem also. 

When viewing the actual cost transactions from the item cost form, I see the cost component and analysis code populated.  They just do not show in the GMF_XLA_EXTRACT_LINES table.

Has anybody seen this before?

What is the cause?  What is the cure?
