To change LOV query of a field on SR form to date LOV (i.e. calendar)

Hi,

I want to change LOV query of a field using form personalization on SR form. Field to be change is getting its value from LOV.

How can I change LOV query so that new LOV will give calendar, so that user can select date as a value. (i.e. which table is to be used for getting calendar)
