Unable to see Account Hierarchy in CRM on Demand usning Client Side Extension(Account Tree- CSE)

I am using the Client Side Extension given at Oracle site. I have uploaded the swf file and followed all the necessary step. I have created a web applet in the Detail page of Account changing the relative path as mentioned in the Document.

But the issue is wheneven I view any account I get the folloiwng flash error

"error retreiveing account information for, no result returned ListOfAccount"

Why is it happening. I have followed all the instruction and recheked as given in the document.
