DBMS_ERROR: 99999 - While cluster is stopping a Oracle DB

We are using Cluster 3.3 U1 for Oracle Databases (10 G).  This weekend  we patched the cluster with Jul 2013 released 3.3 Patches.

 Today we noticed below error when one of the DB was taken offline by a scheduled script. This particular DB went offline successfully but for some reason the  resource went to Faulted state and then went to offline.   All these things happened in a matter of one second.

 Error log noticed inside zone

 Aug 20 04:30:15 regxxxx04a SC[SUNWscor.oracle_server.monitor]:rd04-rg:rd04-inst-DB1: [ID 564643 local7.error] Fault monitor detected error DBMS_ERROR: 99999 DEFAULT Action=RESTART : Monitor detected death of Oracle background processes.
