APEX 4.2.2 Survey Builder App

Tried to run this Survey Builder app, got this error:

The following error has occurred while executing the error handling callback: ORA-06550: line 5, column 38: PLS-00201: identifier 'EBA_SB_FW.APEX_ERROR_HANDLING' must be declared ORA-06550: line 5, column 1: PL/SQL: Statement ignored ORA-06550: line 6, column 12: PL/SQL: ORA-00942: table or view does not exist ORA-06550: line 4, column 5: PL/SQL: SQL Statement ignor

anyone knows what smaple database maybe I need install? or the how to solve the issue?
