changed LOV query using form personalization on SR form

Hi,

I have changed LOV query of field 'PREFERRED LANGUAGE' (CUST_PREF_LANG_NAME) using form personalization on SR form. Also I have changed prompt to 'Action Taken' using folder options.

While selecting value from LOV, it is giving error as 'FRM-41091: Cannot find LOV: Invalid ID.'

Find attached scrrenshots of personalization done and error coming.
