Upgrade OS OEL5 to OEL6 on a RAC 11GR2

 Hi,I would like to ask what's the correct path to follow up for an OS Upgrade, specifically from OEL5 to OEL6 in a RAC environment.

Current Environment:
RAC 11GR2 (11.2.0.3.0)
OS: OEL5 (2.6.18-274.18.1.0.1.el5)

Migrate to:
RAC 11GR2 (11.2.0.3.0) - Stay the same
OS: OEL6 (2.6.39-400.17.1.el6uek.x86_64)

Currently I have a 3 node cluster with OEL5 (2.6.18-274.18.1.0.1.el5) and I would like to upgrade all the nodes to OEL6 (2.6.39-400.17.1.el6uek.x86_64), I tried to add a 4th node with OEL6 to the existing cluster and then remove 1 node, upgrade it and then add it back again. But apparently it doesn't work well since I'm having issues adding the instance to the new node.
