ADI Template database, where does the database name come from?

We recently refreshed our 12.1.3 TEST instance with a clone of the 12.1.3 Production instance.  We also applied the 3rd Qtr CPU Patching as well as the GL RPC Patch 16200528.

 

We have a blank Desktop Integrator/ADI template created prior to the refresh/patching. We double click on the "Source" column and as expected are prompted to login into the TEST instance. After entering user Id/password, click LOGIN and we encounter the error "oracle.apps.bne.exception.BneNoDataFoundException - The document could not be found.  Log File Bookmark: ######."
