Grid Control 11g to 12c (Fresh Install vs Upgrade)

OMS:11.1.0.1.0Repository Database: 11.2.0.1
OS: Solaris Sparc 64bit, 5.10

I have been preparing for upgrading our current 11g grid control to latest 12c version available. Both OMS and repository database are currently running from same server. We have target servers (up to 30) that are mostly Solaris of version 10 and 11. I need to monitor Windows servers with SQL Server databases as well.

How can i run prerequisite checks to make sure both OMS and all target servers satisfy the requirements? Which method is recommended, Fresh vs upgrade? Please point me to appropriate metalink resources, documentation and certification documents etc that are helpful for this task.
