TNS-12640: Authentication adapter initialization failed

 Hello Community,
Hope all are fine. sometimes I see the TNS-12640: Authentication adapter initialization failed in the alert_log of our production database. the error is not on regular basis but it happens now and then. the alert_log shows the following message:-

Fatal NI connect error 12640, connecting to:
 (LOCAL=NO)

  VERSION INFORMATION:
TNS for 64-bit Windows: Version 11.2.0.1.0 - Production
Oracle Bequeath NT Protocol Adapter for 64-bit Windows: Version 11.2.0.1.0 - Production
Windows NT TCP/IP NT Protocol Adapter for 64-bit Windows: Version 11.2.0.1.0 - Production
  Time: 02-SEP-2013 14:14:55
  Tracing not turned on.
  Tns error struct:
    ns main err code: 12640
