Unknown Targets at OEM 11g

Hi,

At my OEM I am facing multiple unknown targets every time. When ever the agent is not able to communicate due to any reason some times its skips to unknown targets list. Also the same issue is been facing by the DB instances, listeners etc....

Please help me whether is there any automate to resolve such kind of issues...

My query is like if a DB is migrated from one version to another version then the ORACLE_HOME path will be updated so in this case the db instance is automatically moving to unknown state, it will be best if the targets at OEM can rediscover the new path automatically.
