Application Diagnostics not showing up

I am trying to add Application Diagnostics responsibility to my user and getting the message that I already have it though I dont see it in the list. When I go to the 'Indirect Responsibilities' tab I see it there with an end date. All the fields are greyed out so I can not remove the end date.

What should I do?
