Hi,
In Opportunity when ever Slaes State is changed/updated then new record will get created in Assessment default
Here Assessment Name field is storing "Opty Name","Assessment Template Name" and "Assessment Id".

How siebel is storing Assessment Name field value when Sales Stage is changed.
