Requisition Error with Activity " Update Action History(Expect Response)

 Dear All,we are having an issue with our Requisition approval hierarchy whereby requisitions error out after being approved by  a person in a particular level in the hierarchy. We have 8 Requisition approval hierarchies and all the approvers at this particular level have the same issue. I am attaching a screenshot to better explain.
