Files skipped during patch

We are in R12.1.1 with 11.2.0.3 upgrading to R12.1.3.

When applying main driver, we noticed few files are skipped with "Turning off the following actions as it is not a concurrent processing server node" and "Turning off the following actions as the file should not be present on-site".

We have 2 concurrent node and 2 forms/web node with Shared APPL_TOP configured.

When using Shared APPL_TOP configuration, I think applying patch in one node will take care and we need not apply the patch in all appl node.

When the same patch was applied in single node instance all files are copied. Any help is much appreciated.
