Receipt Application Forms Personalization

Has anyone created a forms personalization on the receipt application form to limit the values that are returned in the LOV for a receipt write off? I have this requirement and am struggling to get the right trigger in my configuration. I am using Trigger Event 'WHEN-NEW-ITEM-INSTANCE' and trigger object 'APP_FOLDER.REC_ACTIVITY_NAME'.
