Automatic Assign responsibility using Self Servive approval

Dear All,

We have the below requirement....

1) we need to have a Cusotm Self Service form in which we can see all the Oracle responsibilities.

2) User will select the responsibility in this form by selecting a check box..

3) Then it should go for an Approval using AME setup.

4) After approvals, the responsibility needs to be assigned to the user login automatically.

Pls. help me i.. how can I meet this requirment, I have no idea.. pls send me the steps in details...
