XML Template Not Including Latest Text

We have a PO on production that we've updated an amount in an attached note but when we do the View Document it still shows the old amount.  We've also noticed that when we make a change to the xml template and upload it into EBS then View the PO it's not the new uploaded template.  Does it get cached?  How do we ensure the latest text is used and the latest uploaded template is used as well?  It's critical that the template is the latest and uses the latest text.
