XML pulblisher doesn't insert all fields

a- I have developed a Custom report via oracle report builder (RDF format)
b- then i export the report RDF file to XML type

c- then i upload the XML file through oracle XML publisher desktop program (which is adds-ins on in microsoft  office word 2007)
  and then i design the report template interface and save it as RTF format  
d-finally i deployed it to the server by the responsibilty "XML publisher"

the issue i have is in step "c"  when i insert all fields it pops up that error (check attachment file named "insert_fields_error")
    i cannot insert all fields !!
