Add a path to MPxIO LUN

Hi all,

I hope someone can put me in the right direction and give some advice on a problem I have with a server connecting to a switched SAN.  Please forgive my ignorace on this topic, I am not overly familiar with SAN and Solarsi 10 storage commands and techniques.

It appears that one of the LUNs on this server is missing a path, but every article i find on the topic is teaching me how to REMOVE a LUN.  I don't want to remove it, i want to add it to multipathing. 

If I perform a cfgadm, all my disks show as being connected, configured, unknown and I have no failed disks or connectivity issues.  If I perform a format command, i get a device on target /dev/rdsk/c1t500A09818AF276E7d6s2 which should actually not be there and should be part of a multipathed LUN:
