upgrade 12c r2 to 12c r3 fails opss and mds

I am upgrading to R3.  MDS schema fails at first shot, but a retry is successful. But then the installer fails at the OPSS stage.

This is being investigated by MOS, but if anyone in the community has an idea, you are most welcome to suggest a solution.
