Oracle's recommendation for ESUs & POCs?

What is Oracle's recommendation for staying current with ESUs?  Do they recommend taking all ESUs every 6 months or year, or simply waiting for an update/upgrade?

Also, what is Oracle's policy concerning when they are willing to create POCs?
