E1 Pages: User Assigned Multiple Roles, each role having different E1 Pages, *ALL not displaying all

2 Issues for E1 Pages:
1)  I referenced the link: 3360402

and What Is The Purpose of the UseRoleHierarchy Setting in the JAS.in? (Doc ID 953552.1)
and E1 PAGE: Published E1 PAGES not Merging when Signed on as *ALL (Doc ID 1501956.1)

I've updated the UseRoleHierarchy=true and rebooted the server.  My E1 Pages in 9.1 are still not merging.  Any suggestions?  I can select per role at logon and see the appropriate E1 Pages (so I know the individual E1 Pages are functioning.) 

2)  Also, how does the web client choose role hierarchy?? (per attachment)  In this situation, *ALL is selected at logon, but the E1 Pages displayed are for the role showing at the top of the list (per attachment).  But in my "Role
