RMA Lines shipped but OHQ not flushing

  Hi Gurus,
I recently encountered two scenarios, I am not sure if I should mark both in single post. Can you please suggest on these if anything can be done from functional end to fix this.

a) RMA Shipped and Closed but reservations exists. 
               There are many RMA Orders which are received and shipped back to customer, but I can still see the OHQ of Quantity on hand in Staging subinventory. There are no Open Transaction or records stuck in Transaction Open interface. What can be the problem.

b) I have a PTO Model Line on order which did not create any Workflow Activity list and is stuck in booked status. All its children successfully progressed. How can I handle such a situation.. What should I do now ?
