How to disable Custom Code in Blackout Phase for Upgrade to R12?

Dear all,
In the Checklist for Process Manufacturing Migration from Release 11.5.10 to R12.1 or Higher,
> 3. Blackout phase – Prior To Upgrade Patch Installation
> 3.1 Disable any and all custom code and or triggers before migration.

Please can you explain this step! Where and how it should be accomplished?
