How can I link forecst vs demand?

I believe system is calculating wrong demand based on our judgement. Can you tell me how can I link demand vs Forecast entered. If you can provide me a report or an SQL which tells me that the demand order is generated against which forecast than I can tell for sure what is went wrong and why Plan order demand is too low. I need some sort of reconciliation report like Forecast Item and then list me Item used in Forecasted Item:

Reconciliation report should be in the following format:

Forecast Item                          BOM (Items Used in Forecasted Item)
-----------------                          ----------------------------------------
