ORA-20000: ORA-00955: name is already used

Hi There,
I have upgraded the  DB from 11.1.0.7 to 11.2.0.3.4.It was sucessfull.

While doing Post upgrade  steps "Recreate grants and synonym for apps" then i go following error.

FAILED: file adappsgs.pls on worker  1

ERROR at line 1:
ORA-20000: ORA-00955: name is already used by an existing
object:create_grants_and_synonyms(1,PO,APPLSYS,APPS): create_base_gs(PO,APPS):
