Notification for Fast-Start Failover Occurred does not send email

I'm running 11.1.0.1 Grid Control on windows 2008, databases are 11.2.0.2 on Windows 2008 R2 64-bit, basic config is primary and 1 physical standby that is the fsfo target. I have configured a notification rule to have an email sent for the Fast-Start Failover Occurred metric but it does not send an email when i trigger a fast-start failover. I do get emails for other conditions in that same notification rule such as Redo Apply is stopped. So i believe the email part is working and the notification rule itself is working.
Does anyone know of a bug that requires a patch or a specific configuration that is required for this to work?
