R12.1.3 : unable to open Invoices batch form

Hi,
We recently upgrade to R12.1.3 in one of the test instances.  We are unable to open the form   Invoices batches due to following error

ORA-01403:  No data found

Navigation:

Payables inquiry  -->  invoices --> invoices batches.

Any help is appreciated.  Thanks

FRM-40735 : WHEN-NEW-FORM-INSTANCE trigger raised unhandled exeception ORA-04045.
