Journal upload error

The user did  a clean PC Boot.  She opened Excel, opened an ADI template which had previously been uploaded successfully.  She made an adjustment to that template (adding debit and credit values) selected the “Upload” function within the Oracle menu in Excel… received the logon prompt, and subsequent responsibility selection.  Clicked through the template upload prompts, and then at time of “Upload” received the message below.

if we had an error message, diagnosing this would be much easier
