Got FRM: 41050 error while querying item in item view only form

We have a custom item view form, which is set as query only. When we query some particular item (e.g.ATT.100006476), we will get Forms message FRM-41050: You cannot update this record. You can click Ok and proceed to view the info you want to see. Then when user tried to close that window, they will get another message: Do you want to save the changes you have made?
The message above only happens to some particular item like ATT.100006476 but not all the items. The error message and form setup are attached.
