How to add a database to OEM

I have created a couple of new databases on the server that is already in OEM but the new databases did not automatically showed up. I went to Targets->Databases and clicked on Add but it asked me for the host name to add. The hostname has already been added and OEM agent is installe don the server. What do I need to do for the new databases to show up in OEM?

The version of OEM is 12c
