Workorder completed twice

We are on 12.1.3 and noticed that one discrete work order got completed twice and the over completion tolerance is  to 0 .It seems to have to happened only for thsi one WO and The two completion transactions are about a minute apart .We have checked the job was not over completed but noticed that the completiion_transaction_id in mtl_material_transactions is blank for this one addtional completion transaction.Would that be the problem?r Is there anything else we can check to identify why this job was completed twice.Our transactions are processed in the online mode.
