SQLT XTRACT 12.1.03 Report: sqlt_s66506_main.html Having error

Hi,

While running

cd sqlt/run
# sqlplus apps
SQL> START sqltxtract.sql 0w6uydn50g8cx sqltxplain_password

I had checked all  Security Model of document 215187.1 SQLTXPLAIN (SQLT) 12.1.03 October 10, 2013
sqlt_s66506_main.html file having following error.

Review log and fix following errors:
sqlt$a: *** i:"EXPLAIN PLAN FOR" this SQL errored out when executed connected as "ORSFHR_PRD". Always execute SQLT connected as the application user.
sqlt$a: *** i:ORA-00905: missing keyword
sqlt$a: *** d:ORA-20000: TABLE "SYS"."DUAL" does not exist or insufficient privileges
sqlt$a: *** d:could not export stats for table = "DUAL", owner = "SYS"
sqlt$a: *** d:ORA-20000: TABLE "SYS"."ACCESS$" does not exist or insufficient privileges
