Personalization in GUI form not saving

Hello,

Some personalizations will not save in the grants award inquiry form that we have. When trying to disable a personalization, nothing happens (I did save). If I change the personalization from true to false, it works. If I create a new personalization, the new setup won't work.  When I click "apply now" it shows everything correctly as I would expect based on the personalization setup. However, when opening the form, the personalizations are all removed despite me clicking save before closing the personalizations form. Any suggestions are appreciated. We are on 12.1.3.
