Database Connection Error : TNS Connection Lost after starting the Concurrent Manager

 Hi All,
>I have done the db refresh from RMAN backup and successfully ran auto config on apps tier and db tier.

> Application - 12.1.3, Database - 11.2.0.2 , OS - AIX 6.1

>If i start the concurrent manager then error occurs while accessing forms and error in the login page if the concurrent manager is up and running.

> One of the error : Unable to generate forwarding URL. Exception: oracle.apps.fnd.common.AppsException: oracle.apps.fnd.common.PoolException: Exception creating new Poolable object

>When i stop adcmctl.sh, Application and Forms works fine without database connection lost...When i start adcmctl.sh then the issue starts in login page, concurrent and also while accessing forms.
