I get an error FRM-4073 ON-INSERT When I try to insert a new revision in the Form BOMFDBOM

When I try to create a new BOM revision for an SKU ( using the form BOMFDBOM ) I get this error:

ORA-01403: no data found

--------------------------------------------------------------
FRM-40735: ON-INSERT trigger raised unhandled exception ORA-04062.

I attach the form.

I try to found some patch or note in metalink but I didn't found.  before create an SR I would like to know if someone have any idea.

I guess it must be an Form compilation issue but I don't know how compile this kind of forms
